Passport Application Acceptance Service

Appointments are required for each person getting a passport and take approximately 15 minutes per applicant.

What to Bring

  • Proof of U.S. Citizenship
    • Previous Passport
    • Certified Birth Certificate With a Registrar's Seal
    • Naturalization Certificate
    • Citizenship Certificate
  • One Photograph
    • Taken within the past two months
    • 2x2 inches, front view full face on white or off-white background
    • Photograph can be taken at the appointment for $20
  • Proof of Identity
    • Previous Passport
    • Certificate Of Naturalization or Citizenship
    • Valid Driver's License or DMV identification card
    • Official military identification card
  • DS-11 Form
    • Available at all Livermore Public Libraries
    • Available on the Internet at: www.travel.state.gov
      Must be signed in the presence of the passport application acceptance agent
    • Type or print clearly in black ink only
  • Children Under 16 should click for U.S. Passport Requirements
  • Check or money order for passport and handling:
    • Adult First Time Applicants (Age 16 Years and Older)
      • Adult Passport Book and Card: $140 for a 10 year passport book and card (made out to U.S. Department of State) + $25 execution fee (made out to the Livermore Public Library)
      • Adult Passport Book: $110 for a 10 year passport book (made out to U.S. Department of State) + $25 execution fee (made out to the Livermore Public Library)
      • Adult Passport Card: $30 for a 10 year passport card (made out to U.S. Department of State) + $25 execution fee (made out to the Livermore Public Library)
    • All Minor Applicants (Under Age 16)
      • Minor Passport Book and Card: $95 for a 5 year passport book and card (made out to U.S. Department of State) + $25 execution fee (made out to the Livermore Public Library)
      • Minor Passport Book: $80 for a 5 year passport book (made out to U.S. Department of State) + $25 execution fee (made out to the Livermore Public Library)
      • Minor Passport Card: $15 for a 5 year passport card (made out to U.S. Department of State) + $25 execution fee (made out to the Livermore Public Library)
  • $60 expedite fee (if needed) (made out to U.S. Department of State)
    Social Security Number is required on all applications.
  • $20 fee for photograph (if needed) (made out to the Livermore Public Library)

Normal processing time is 6 weeks.
